* Fixed an issue where users would receive an error when trying to submit a Staff Application

- Removed 'Email Address' field from Staff Application form

- Removed 'Cover Letter' field from Staff Application form

- Removed 'Website' field from Staff Application form

- Removed 'Resume/CV or Qualifications' on field form for Helper, Moderator, Global Moderator, Builder

- Removed Killerteddy
